I release Eliminate Chaos and their workers, of any and all liability for injury or damages that may occur as a result of the loading, unloading or transportation of any personal property or goods.

I understand that Eliminate Chaos, nor their employees or contractors, are not responsible for damage or injury caused by the contents of my property while in transport, including injury caused by or items damaged during a car accident or theft. This includes Eliminate Chaos company vehicles and any personal vehicles owned by employees, contractors or client personal vehicles that are used for the transportation of these goods or personal property.
