I give my permission to the Specialists at Eliminate Chaos, LLC to photograph my project. I understand that Eliminate Chaos will not use my name in conjunction with these photos and will protect my confidentiality as stated in the Eliminate Chaos Service Agreement.

I understand that I waive my right to inspect or approve the finished product or other copy of said photographs that may be used.
