I authorize Eliminate Chaos to transport any items I have chosen to donate and will not hold Eliminate Chaos liable for these items after they leave my premise.  I understand I will be given a receipt for tax deduction purposes and I am responsible for itemizing and filling out the appropriate information on the receipt.  I understand that in no way does this service benefit Eliminate Chaos or its employees.

If I choose to have Eliminate Chaos recycle or donate any electronic equipment, such as a laptop, desktop PC, cell phone, tablet, iPad or any device that contains personal information, Eliminate Chaos will not be held responsible if the device is not properly wiped clean.
